\- the DVD bonus of the anime Ano Hi Mita Hana no Namae o Boku-tachi wa Mada Shiranai. (shorten Ano Hana)
\- ep 11 (finale) of the anime (UTW's translation is used here).

The song is Secret Base - Kimi ga Kureta Mono (10 years after Ver.). It's first performed by ZONE, then recovered by SCANDAL and also the anime cast (Ai Kayano as Menma, Haruka Tomatsu as Anaru, and Saori Hayami as Tsuruko).
