Anna Marchant (Milla Jovovich) is een lerares op een basisschool.
Ze wordt aangevallen door een seriemoordenaar die alleen bekend staat als 'Tearjerk Jack'.
Ze overleeft de aanval maar houdt er een neurologische afwijking aan over genaamd gezichtsblindheid of prosopagnosia.
Alleen in staat om stemmen te herkennen, vecht ze om zich aan te passen aan haar afwijking en de voortdurend veranderende gezichten van de mensen om haar heen.
Samen met de rechercheur Kerrest probeert ze de identiteit van haar aanvaller te achterhalen.
Ondertussen is de moordenaar vastberaden om deze potentiële getuige uit de weg te ruimen.
Alleen Anna heeft zijn gezicht gezien, maar het kan iedereen zijn.
