Subs is OCRed(?) from a Hardsubbed KissAnime rip, looks like DVDrip considering its colour.
I tried to make everything as close to the original as possible, except for OP and ED.

For the OP and ED,
I didn't do the romaji cos I think it would be redundant,
I changed the font,
and I placed them under the Japanese lyric instead of above.

English translation for some lines are way longer than the spoken Japanese, you gotta read fast.
