Après les révélations du final de la première saison, John et Quiet se retrouvent à participer au tournoi mortel de Twisted Metal, un sinistre derby de démolition organisé par un homme mystérieux connu sous le nom de Calypso. Alors qu'ils tentent de survivre à l'assaut de nouveaux ennemis dangereux et de visages familiers, dont le clown meurtrier Sweet Tooth, les choses se compliquent pour John lorsqu'il retrouve sa soeur perdue de vue depuis longtemps, la justicière Dollface.
